Well, it can mean honestly and sincerely, "May God bless him."
Especially when speaking of the dead or dying---or of someone
undergoing difficulties.

But, if said snidely, it means, "What a dumb-ass!"

I once read a hilarious essay regarding the expression "bless her heart" and I agree with its conclusion: this means the person in question is hopeless...only God really has the patience to deal with them.. This is code-speak for a lot of unkind thoughts. I have had my heart blessed numerous times! Ha, ha....

Me too! The length of my hair, my rather eccentric style of dress, my
kid wearing shoes on the wrong feet or goggles and fairy-wings, the
fact that I live in an area called Dimal - that's really the name! The
bank ladies, especially, love to bless my heart. I have heard it said
kindly - once wrt a death and once when I mentioned my stepson moving
in with me. But usually its the meanest thing a "respectable church
lady" can say in public.
